There won't be many Israeli casualties in this attack if any at all.

1. Israel has been sincerely hoping that Iran would attack her. They have been provoking Iran again and again.
2. Israel would have received the missile attack warning from US satellite based missile launch detection system and its own spy network in Iran.
3. Entire Israeli population would have sheltered in their bunkers.
4. Israeli and American missile defences would have destroyed major portion of this missile barrage.

Hopefully Iran has similar measures in place. Because Israelis are coming, and not just to destroy Iran's defence installation, but Iran's nuclear infrastructure also.

Realities:

- Whatever depth Israel has, Iran has 100x as much.
- When Iran launches these strikes, 50-100% of the Israeli population is impacted due to the tiny size of Israel. When/if Israel strikes Iran, <10% of the population will even notice.
- If Israel launches an attack against Iran, Iranian ballistic missiles will land in Tel Aviv before the Israeli jets can return to Israel (if they return).

Iran took the initiative. It should continue to use these missiles while it can. This attack was more successful than anyone could imagine. How many ABM interceptors does Israel have left?

I would say that it's clear that Israel never fully recovered from the April attack.

This probably has made their situation even worse.

The only way to get better interception rates is to get the US directly involved in a heavy way, which the US is reluctant to do.

Reasons for not having so many casualties:

The impacted places were military targets and civilian/residential areas were not targeted, at least not on purpose

The impacts must have occurred in dispersed and uninhabited areas, without crowds (such as air bases), causing merely material damage

The warning systems give a good advance warning for Israeli citizens to hide in bunkers or hide in some fortification
