BLA always blow something up, Railway bridges and tracks along with gas pipelines are the favourite things to blow up. The Railway network is frequently targeted in Balochistan, but never at this scale.

Every train has Railway Police on it, a few Railway Police officers cannot prevent a larger hostile force from attacking or taking over the train.

They should have at least a QRF to call. Heilborne special forces that can at least repel an attack and laser designate concentration of militants for the Air Force to bomb.

This is a very open rebellion. The BLA used to have recruitment issues. Not anymore. The planning that went into this attack is pretty massive. These people also had help from external powers.
