You're confusing a nuclear reactor with a hydrogen-fusion reactor. In a nuclear thermal propulsion concept, the reactor does not burn hydrogen as nuclear fuel. To clarify the distinction:
Fission reactor: Uses uranium (or another fissile material) as fuel to generate heat.
Hydrogen propellant: Absorbs that heat and is expelled to create thrust. It is not undergoing a nuclear reaction.
Fusion reactor: Attempts to fuse hydrogen isotopes to release energy. This is a completely different concept and is not required for nuclear thermal propulsion.
