Central Retinal Vein Thrombosis (CRVO) happens when the main vein draining blood from the retina becomes blocked by a clot — essentially a stroke of the eye. The causes are best understood in 4 major categories:
1.Systemic Vascular Risk Factors (Most Common)
These account for the majority of CRVO cases, especially in patients >50 years old:
Hypertension#1 risk factor
Diabetes mellitus
Hyperlipidemia
Atherosclerosis
Smoking
Obesity
Advanced age
Why?
Atherosclerotic thickening of the central retinal artery compresses the adjacent central retinal vein at the lamina cribrosa → turbulent flow → clot formation.
2.Hypercoagulable (Thrombophilic) States
Especially important in younger patients (<50) with CRVO:
Factor V Leiden mutation
Prothrombin gene mutation
Protein C or S deficiency
Antithrombin III deficiency
Antiphospholipid syndrome
Hyperhomocysteinemia
Polycythemia vera
Essential thrombocythemia
Oral contraceptive use / estrogen therapy
Think of these when:
CRVO occurs in a patient with no hypertension or diabetes
3.Ocular Causes
GlaucomaStrong ocular risk factor
Elevated intraocular pressure
Optic nerve head crowding
Ocular inflammation (uveitis)
Trauma
Raised IOP reduces venous outflow → promotes venous stasis.
4.Inflammatory / Systemic Diseases
Less common but important:
Vasculitis (e.g., SLE, Behçet disease)
Sarcoidosis
Temporal arteritis
Severe dehydration
Malignancy-associated hypercoagulability
Clinical Pearl (Exam & Real Life)
Age Group Most Likely Cause
> 50 yrs HTN / DM / Atherosclerosis
< 50 yrs Hypercoagulable state
Any age + glaucoma IOP-related venous stasis
